Dear Mr. Hall,

In accordance with Part III, Section 12.c of the Permit, this letter is serving as confirmation of a telephone report of an effluent limitation exceedance made to the State Emergency Response Commission on 2/10/95.

1. The limitation which has been exceeded: The time limit (tvo hours) for discharge of total residual chlorine (TRC) from outfall 3IB00016004, the discharge tunnel. Free available chlorine was measured at 0.0 ppm at the final discharge point near the end of the event.
2. The extent of the exceedance: A two hour and fifteen minute discharge of TRC resulting from approximately 2 gpm of 0.8% sodium hypochlorite entering the plant service water system.
3. The cause of the exceedance: A mispositioned switch ("on" vs.

" auto").

4. The period of exceedance: From 2135 to 2350 on 2/9/95.
5. The anticipated time the exceedance is expected to continue: The release was stopped at 2350 on 2/9/95.

6. Steps being taken to reduce, eliminate and/or prevent recurrence of the exceedance: A Human Performance Enhancement System evaluation is being conducted along with a detailed investigation. The results of both evaluations vill be used to determine specific actions necessary to prevent recurrence.
