Twinsburg, Ohio 44087 Ladies and Gentlemen:
This letter is submitted in accordance with the National Pollutant Discharge and Elimination System (NPDES) permit number 3IB00016*ED, Part III, Item 12.C. On August 18, a telephone notification was made to report an NPDES permit noncompliance. The following information fulfills the permit requirements.
- 1. The limitation which has been exceeded.
Chlorinated water bypassed the permitted discharge point (entered a storm drain).
- 2. The extent of the exceedance.
Approximately 200 gpm for 40 minutes of water containing 1 ppm chlorine.
- 3. The cause of the exceedance.
[o Equipment malfunction - valve leakage.
- 4. The period of the exceedance, including exact dates and times.
The incident began on 8/17/99 at 09:10 and ended on 8/17/99 at 09:50.
- 5. If uncorrected, the anticipated time the exceedance is expected to continue.
Not applicable. Chlorination terminated. Valve isolated to allow for chlorination until valve l
repairis accomplished.
