In accordance with 40 CFR 302, Ohio Administrative Code, Section 3745-65-56, and Ohio Revised Code, Section 3750.06, this letter serves as confirmation of a telephone report of a reportable release of sodium hypochlorite. The report was made to the Ohio State Emergency Response Commission and the OEPA on 3/20/99 at 14:30.

Per the Ohio Administrative Code the following infonnation is provided.

Approximately 4000 gallons of an aqueous solution containing 12.5% (g sodium hypochlorite leaked from a tank to a concrete containment.

sodium hypochlorite - Approximately 500 gallons leaked to a concrete secondary containment.

5. The extent ofinjuries,if any:

6. An assessment of actual or potential hazards to human health or the environment, where this is applicable.

None. The material was entirely contained in the secondary containment structure designed to contain the tank contents,

7. Estimated quantity and disposition of recovered material that resulted from the incident:

l 500 gallons of sodisT nypechlorite in 4000 gallons of aqueous solution was recovered and pumped to I containers. The material will be used as intended in the plant service water system.

Per the Ohio Revised Code, Section 3750.06, the following information is provided.

1. Actions taken to respond to and contain the release:

The leak was entirely contained in the concrete structure surrounding the tank. The spilled material was pumped from the containment structure to containers.

2. Any known or anticipated acute or chronic health risks associated with the release:

f None.

3. Advice regarding medic attention necessary for individuals exposed to the substance released:

None.

4. A summary of all actions taken by the owner or operator to prevent a recurrence of the release:

An investigation is underway to determine appropriate and permanent corrective actions necessary to prevent recurrence. 'Ihe failed supply line has not been repaired or replaced at this time. The tank will not be refilled until the supply line has been repaired.

5. Such other information as is required by rules adopted under division (B)(1)(f) oi'Section 3750.02 of the Revised Code:

None.
