In accordance with 40 CFR 302, Ohio Administrative Code, Section 3745 65 56, and Ohio Revised Code, Section 3750.06, thic letter serves as confirmation of a telephone report of a reportable release of trichloroethylene. The report was m .de to the Ohio State Emergency Response Commission and the OEPA on 1/2398 at 0418.

None. The area was isolated and clean-up was performed by a c,ualified hazardous materials team.

7. Estimated quantity and disposition of recovered material that resulted from the incident:

. Approximately 70 gallons were recovered and handled in accordance with the site Chemical Release Contin;,ency Plan. Disposal will be in accordance with RCRA regulations. A licensed Treatment, Storage and Disposal Facility will be utilized.

Per the Ohio Revised Code, Section 3750.06, the following information is provided.

1. Actions taken to respond to and contain the release:

SCBA in an attempt to isolate the leak. This was unsuccessful. An emergency response vendor was summoned

- to provide a qualified hazardous materials team for the purpose of entering the hot zone and cleaning up the spiil. Approximately 120 gallons leaked of which 70 gallons were recovered. He remaining 50 gallons volatized.

2. Any known or anticipated acute or chronic health risks associated with the release:

None.

3. Advice regarding medical attention necessary for individuals exposed to the substance released:

None.

4. A summary of all actions taken by the owner or operator to prevent a recurrence or the release:

An investigation is underway to determine appropnate and permanent corrective actions necessary to prevent

, recurrence. He failed gauge has not been repaired or replaced at this time. The portion of the system where the
