The telephone report was made to the Ohio State Emergency Response Commission at 1659 on February 5, 1996. A similar report was made to the Environmental Protec' ion Agency on February 5, 1996 at 1709.

The initial report was made due to a thin sheen of oil observed in site storm drains and on a storm drain impoundment pond. The sheen was observed in conjunction with failure of a bushing on a transformer. The failure resulted in a fire that was extinguished by water sprayed from a fire protection system. Based on the information available within the 30-minute notification period, it was concluded that the oil creating the sheen was from the failed bushing.

During the investigation of the failure, it was noted that no oil was lost from the bushing. It was also noted that the volume and velocity of fire protection system water entering the oil separator that receives '

i flow from the transformer area was sufficient to mix the contents and allow release of the thin sheen of oil. l As provided belov, the sheen was contained in an impoundment pond on the  ;

plant site. It was degraded by freezing overnight and exposure to i sunlight the following day.

A sheen of oil was released from an oi* 'entrator. The flow from the oil separator is routed to a site s.orm drain system, which empties into an impoundment pond. At the time of the release, a very thin sheen was noticed in the storm drains and on the impoundment pond. The sheen was contained in the impoundment pond.

The sheen was degraded by freezing overnight and exposure to sunlight the following day. No material was recovered.

The following information is provided in accordance with the Ohio Revised Code, Section 3750.06:

1. Actions taken to respond to and contain the release:

f No actions were required to contain the release. The plant is designed to contain releases of this nature. The flow from the oil separator is directed to a site storm drainage system, which flows into an impoundment pond. The pond is equipped with a dam and skimmer plate that prevent floating contaminants from being discharged from plant property. The thin sheen resulting from this release-vas contained in the pond. It was degraded by freezing overnight and exposure to sunlight the following day.
