In accordance with Part III, Section 12.c of the Permit, this letter is serving as confirmation of a telephone report of an effluent limitation exceedance made to the State Emergency Response Commission on 3/23/95.

1. The limitation which has been exceeded: The time limit (two hours) for discharge of Total Residual Chlorine (TRC) from outfall 3IB00016004, the discharge tunnel. Free available chlorine was measured at 0.0 ppm at the final discharge point near the end of the event.

2. The extent of the exceedance: A 23.47 hour5.439815e-4 days <br />0.0131 hours <br />7.771164e-5 weeks <br />1.78835e-5 months <br /> discharge of TRC resulting from approximately 2 gpm of 0.8% sodium hypochlorite entering the plant service water system.

3. The cause of the exceedance: A failed valve.

4. The period of exceedance: From approximately 0030 on 3/22/95 to 0002 on 3/23/95.

5. The anticipated time the exceedance is expected to continue: The release was stopped at 0002 on 3/23/95.
