Dear Mr. Keppler:

On January 25, 1982, Mr. T. D. Geib of Public Service Company of Indiana Inc. , (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). Review of the Nuc1 car Steam Supply System (NSSS) support steel document packages supplied by Lakeside Bridge and Steel Co.

revealed that records were missing from several packages and the quality of the affected supports was therefore indeterminate.

PSI conducted a review of NSSS support steel document packages and retrieved many of the missing records from files at Lakeside Bridge and Steel's shop.

Review of certain retrieved radiographic records necessitated the weld repair of three pieces of support steel that had been delivered to the site.

Had those pieces requiring repair remained uncorrected, their ability to support the NSSS during a design basis event may have been impaired.

In the last interim report dated July 2, 1982, PSI identified 307 weld seams which did not have all the required records of Nondestruc-tive Examination (NDE). Ten additional weld seams have since been iden-tified for a total of 317. Of the 317, 283 involved the sequencing of surface examination of weld seams in A-588 material.Section III of the ASME Code allcws materials designated as P-No. 1 to be examined before or after post weld heat treatment (PWHT). Code Case 1644-2 authorized use of A-588 material but did not provide a P-No. classi-fication. It was determined by the owner, architect engineer and Lakeside Bridge and Steel Co. that A-588 material should be treated as a P-No. 1 material. Surface examination was therefore performed prior to PWHT. A later revision of the Code Case (1644-9) identified A-588 as equivalent to a P-No. 3 material. This later designation resulted in the identification of 283 weld seams as not having records of surface examination after PWHT. Subsequent to the July 2, 1982, interim report, PSI's Authorized Inspection Agency advised PSI that the later Code Case is not binding on material fabricated before okOO56 t'.u. box 2 ,D....... ,n, Indiana 47162 812. 289.3000 NOV 2 21982

Of the remaining 34 weld seams, 30 involved missing records of Code required Ultrasonic testing. Of the 30, 27 were successfully examined on site and adequate documentation has been submitted for three seams which indicates all inspections were completed.

The final four seams involved missing surface examinations which were successfully examined on site.

No further corrective action by Lakeside Bridge and Steel will be necessary since furnishing of the NSSS Support Steel is complete.
