Dear Mr. Keppler:

f On April 21, 1983, Mr. L. G. Yeager of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). Westinghouse notified PSI of a potential for improper operation of DS-416 reactor trip switchgear undervoltage attachments. A design discrepancy involving the s.,roove in the shaft receiving the retaining ring increases the potential for improper operation, thereby creating a condition wherein the reactor trip switchgear might not open on automatic demand from the reactor protection system.

Westinghouse has committed to replacing the undervoltage attachments on DS-416 reactor trip switchgear which they supplied to PSI for the Marble Hill project. Westinghouse says the new attachments will have modified grooves which will correct the previously noted design discrepancy.
