Dear Mr. Keppler:

On August 5, 1982, Mr. T. D. Geib of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). During a field modification / inspection, Westinghouse discovered six sheared pinion keys in Limitorque Model SB-0 valve motor operators. The sheared pinion keys could result in the failure of the affs :ted valves to perform their intended safety function. This operator is used on a number of valves in safety-related systems supplied by Westinghouse.

In PSI's letter of January 18, 1983, you were informed that Wantinghouse had committed to supply PSI with a list of affected motor operators and provide replacement pinion keys for them. Westinghouse has now informed PSI that based upon extensive analyses, metallurgical evaluations and performance tests on the affected pinion keys, Westinghouse and Limitorque have concluded that the pinion key failures were a result of circumstances peculiar to SB-0-25 Limitorque operators installed on valves manufactured at Westinghouse Electro-Mechanical Division (EMD). Therefore, corrective action is only required on the SB-0 series Limitorque motor operators installed on Westinghouse EMD manufactured valves. Westinghouse has further stated that there were no such valves shipped to Marble Hill from Westinghouse EMD.

Based on this inforaation, PSI determines this item not reportable under 10 CFR 50.55(e). If you have any questions regarding this matter, please contact me at your convenience.
