This is to report potential defects as described below.

For your information Louis Allis is the successor to Beloit Power Systems for nuclear diesel generator control panel switchgear spare parts.

Problem Potentially defective operation in service of Synchronizing Speed Switches used in safety related auxiliary generating equipment (standby diesel gen-orators) at the listed power stations could prevent operation of the equipment.

(See attached list).

i Investigations by Louis Allis OA with the manufacturer - Synchro-Start 1

Products, Skokie, IL has disclosed the following information.

The in-vestigations were made upon Louis Allis rejection of two model ESSB speed switches for speed switch relay high and erratic contact re-4 sistance.

Report by Synchro-Start that they have been experiencing a high

rate of Potter Brumfield R10 relay defects and test failures over an extended period of time.

Documented analysis and corrective action report by Potter & Brum-field provided by Synchro-Start during an investigative limited survey at their plant.

The analysis reports on a sample of 13 re-1 lays selected from 43 defective relays sent to P&B in January J

1986.

P&B report states that design of relay prior to August 1985 could cause phenolic dust particle contamination of relay con-tacts that could result in intermittent closures.

The report also i

shows R10 relay characteristics that can permit defective relay i

operation if proper manufacturing controls by user are not fol-lowed:

Corrosion on relay armature hinge caused by cleaning solvents used during manufacture of the speed switch (relay is not of scaled construction).

Tension on coil wires that could lead to breakage during i

service if P&B specified maximum allowable 020" coil lug movement occurs in manufacture.

i Synchro-Start has informed Louis Allis that because of their reported i

problems with R10 relays they have discontinued their use in the ESSB l

l speed switch.

Limited survey at Synchro-start further showed that standard performance l

tests and control does not adequately test for the potential defects.

Corrective.Tculon Louis Allis Drives & Systems Division to investigate and issue report which will Cefine corrective action.

Report will be issued by 26 Sep-tomber 1986.

Advice 1.

Switches should be checked at first available opportunity for pos-j itive contact function when the relay is energized and deonergized.

A.

If the speed switch is not installed, the contact operation may be checked by placing low resistance ohmmeter across proper terminals of speed switch.

Observation - Should be no indication of erratic operation of contacts when energized or deenergized.

Action - Remove and replace if erratic operation is observed.
