Dear Mr. Keppler:
On August 18, 1982, Mr. T. D. Geib of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). This problem involves embedded plates in the primary shield wall having deformed wire anchors missing or the welds between the plate and anchor being. cracked. These plates had been installed prior to the cessation of safety related work in August, 1979.
1 Tests performed by an independent laboratory to determine the failure mode of the cracked welds have been completed and the results analyzed.
The results of these tests indicate that an axial tensile stress applied to the welds was the primary cause of failure. PSI is now working to determine the source of the stress.
This letter is intended to fulfill the requirements of an interim report as defined in 10 CFR 50.55(e). A further report will be filed on or before September 2, 1983. If you have any questions regarding this matter, please contact me at your convenience.
