Dear Mr. Keppler:
On October 21, 1983, Mr. L. G. Yeager of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). PSI has been notified that a number of Pacific Scientific mechanical shock arrestors have a potential quality problem. This problem involves the potential malfunction of the capstan spring contained in the Model PSA-1 and PSA-3 Pacific Scientific mechanical shock arrestors. This potential malfunction could have, prevented the shock arrestors from performing their intended safety functions thereby adversely affecting the safe operations of the plant.
PSI has identified the affected shock arrestors supplied to Marble Hill.
All of the subject shock arrestors have been rejected and are being returned to Pacific Scientific. This letter is intended to fulfill the requirements of a final report as required by 10 CFR 50.55(e). If you have any questions regarding this matter, please contact me at your convenience.
