Dear Mr. Keppler:

On July 15, 1983, Mr. S. J. Brewer of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). (Cracks in welds at stiffeners to box beams in the Unit 2 containment building were discovered af ter reinspection of the welds. Required visual inspection found the welds acceptable while supplemental magnetic particle examinations detected the cracks.

Since the magnetic particle examinations are not required and are not always performed, it is possible that the cracked welds could have gone undetected. Had this condition gone uncorrected, it could have affected adversely the safety of operations of the plant during a seismic event.

PSI engineering personnel determined that the weld cracking was due to the welding method being used. It was found that increasing the preheat temperature produced sound welds. The welding method was therefore modified for future use. The cracked areas previously identified were ground out and repaired in accordance with approved procedures.
