Dear Mr. Keppler:
On June 7,1983, Mr. S. J. Brewer of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e) . This item pertains to the lack of quality assurance involvement (f abrication inspection) in the fabrication of Seismic Categorv I HVAC ductwork which does not serve a safety-related function. This lack of involvement is not consistent with PSI's commitment to Regulatory Guide 1.29, Revision 3. PSI has determined that had this problem gone uncorrected, the potential exists for a situation to have arisen whereby a safety-related function could have been impaired during a seismic event due to the failure of non-safety-related equipment.
PSI has defined the scope of the problem and is currently working to determine the necessary corrective action. To date, PSI has initiated changes to contractor specifications, the Project Quality Assurance Manual, and applicable contractor procedures to further implement PSI's commitment to Regulatory Guide 1.29, Revision 3.
This letter is intended to fulfill the requirements of an interim report as required by 10 CFR 50.55(e). A further report will be filed on or before April 6,1984. If you have any questions regarding this matter, please contact me at your convenience.
