Identification of Event Train A and Train B Component Cooling Water (CCW) were cross connected.

Technical Specificaton 3.7.3.1 requires at least two independent CCW loops operable.

i Conditions Prior to Event J

The Unit was in Mode 1 operating at 100% steady state power.

Charging pump IC was tagged out for maintenance. Charging pump 1B '

was aligned to Train B.

. Description of Event At 2105 on 11/21/78, during the review of FNP-1-STP-23.7, " Component Cooling Water System Flow Path Verification Test", it was determined i

that the Component Cooling Water (CCW) supply and return valves for  ;

the IB Charging Pump were incorrectly aligned for existing plant  !

conditions, although the valve positions were as required by I i

FNP-1-STP-23.7. FNP-1-STP-23.7 was written for the assumed condition -

of the IB Charging Pump to be aligned to the A Train. At the time the condition was discovered the IB Charging Pump was aligned to l the B Train since the IC Charging Pump was out of commission. The B ; Train CCW supply and return valves were aligned to the IB Charging Pump. Having both the A and B Train CCW supply and return valves to the IB Charging Pump open cross connected the two trains of CCW.

Technical Specification 3.7.3.1 requires two independent CC. loops operable.

A temporary change was made to FNP-1-STP-23.7 to show the proper flow path for the plant conditions when the IB Charging Pump is aligned to the B Train. The A Train CCW supply and return valves

for the IB Charging Pump were shut at 2115 on 11/21/78 and Train l separation was restored.

i A review of plant documents revealed that on 10/19/78 the trains I had become cross connected when the Assistant Plant Operator performing l FNP-1-STP-23.7 aligned the A Train valves in accordance with the position called for by the procedure. Both trains of CCW were operable during the period that the systems were cross connected.

. t 78-086/03L-0 Designation of Apparent Cause FNP-1-STP-23.7 did not specify 1B Charging Pump alignment to Train B CCW.

Analysis of Event '

Train A and Train B CCW were returned to an operable status within the time limit specified it. Technical Specification 3.7.3.1.

Technical Specification 3.7.3.1 action statement requirements were met. The health and safety of the general public were not affected by this occurrence.

Effect on Plant This occurrence had no significant effect on plant operation.

Corrective Action FNP-1-STP-23.7 was revised to indicate the required flow paths for plant conditions when the IB Charging Pump is aligned to either train.
