Farley Unit 1 Report Date: 11/22/78 Event Date: 10/25/78 Identification of Event A review of FNP-1-STP-124 data indicated that the Filter penetration for the B train of the penetration room filtration system was .07%

filter penetration versus the .05% filter penetration requirement of Regulatory Guide 1.52, Rev.1, July 7,1976, paragraph C.5.c.

Conditions Prior to Event The Unit was in Mode 1 operating at 100% steady state power.

Description of Event Surveillance test procedure FNP-1-STP-124, Penetration Room Filtra-i tion Performance Test, was performed during the period 9/18/78 through 9/22/78. The unit was shutdown (in Mode 5) for a mainten-ance outage at the time. The test results indicated that the filter penetration for the B train penetration room filtration system was .07% penetration (as measured by a DOP test) vs. the

.05% penetration specified in the surveillance test procedure acceptance criteria. Technical Specification 4.7.8.1.e requires that the filter bank efficiency be greater than 99%. The test engineer interpreted this to allow a filter bank penetration of less than 1%. On this basis, a temporary change to the FNP-1-STP-124 acceptance criteria was initiated to revise the acceptance criteria to reflect a 1% penetration limit. The test was completed, and signed off as being satisfactory on 9/23/78. The unit re-entered Mode 4 at 1507 on 10/3/78.

On 10/25/78, a review of the temporary change revealed an apparent conflict in requirements between the Technical Specification noted above and Technical Specification 4.7.8.1.b.2 which refers to the requirements of Regulatory Guide 1.52, Revision 1, July 1976, paragraph C.S.c. This paragraph specifies a maximum allowable i

filter penetration of .05% (as measured by a DOP test). Since this value had been exceeded, the B train penetration room filtration

system was declared inoperable at 1230 on 10/25/78. The unit was in Mode 1 operating at 100% steady state power. It was the intent of the plant staf f to request a clarification of the HEPA filter penetration requirements in order to determine the operability of the B train penetration room filtration system.

. 78-082/03L-0 At 2235 on 10/25/78 the unit tripped. A temporary change to Technical Specification 3.7.8.1 was requested and approved on 10/26/78 to allow the unit to be returned to power while the limiting condition

for operation (B train penetration room filtration system inoperable)
was in effect. The plant filter testing consultant was scheduled to be on-site on 10/27/78 to follow the corrective maintenance and retest.

The B train penetration room filtration system was repaired, satis-factorily retested, and returned to an operable status at 1511 on 10/27/78.

Designation of Apparent Cause The excess penetration was caused by low spring tension on the filter support assembly.

Analysis of Event Technical Specification 3.7.8.1 action statement requirements were met. The penetration room filtration system is only required during the recirculation phase after a LOCA. The A train penetration roca filtration system was operable.

The health and safety of the general public were not affected by this occurrence.

Effect on Plant This occurrence prevented an immediate return to power af ter the unit tripped on 10/25/78. A temporary change to Technical Specifi-cation 3.7.8.1 was required to permit a mode change while under the

limiting condition for operation.

i Corrective Action The spring tension was adjusted and a satisfactory retest of FNP-1-STP-124 (partial) was performed.

A proposed Technical Specification revision allowing a maximum filter penetration of <1% was approved by the PORC and is awaiting NORB approval.
