This report is being submitted in accordance with 10CFR73.71. ,

l On 6-19-89, a self-initiated safety system assessment of the service water system was being conducted. During this assessment, questions were raised concerning the routing of a 24-inch drain line from the service water equipment '

drain sump. This line goes from the SVIS to a discharge area approximately 900 feet outside the SVIS protected area fence. The discharge end of the drain line

vas covered with water and dirt and the end of the drain line was not visible.

1 l An investigation by the plant staff revealed that no barrier existed on either end of the line to prevent access. The size of this drain line exceeds the 96 square inch limit for an unprotected opening. Thus, it was determined that unauthorized entry into the SVIS vas possible through this line.

A Security Force Member was immediately posted inside the SVIS and a thorough search was conducted of the structure. When the discharge end of the drain line was located, a Security Force Member was posted in that area also. Barriers were installed on both ends of the drain line to prevent unauthorized access.

These modifications were completed on 6-23-89. The SVIS has also been valked down to ensure that no similar conditions exist. No safety systems were affected or threatened. 1 l
