Dear Mr. Seyfrit:

On June 25, 1981, T1.ermon Manufacturing Company received a letter from Bechtel Power Corporation, Los Angeles, California informing Thermon Manufacturing Company of a possible deficiency in the Thermon Heat Trace System installation utilized in the San Onofre Nuclear Generating Station, Units 2 and 3. Bechtel Power Corporation also informed Thermon that Southern California Edison had reported the incident to the Nuclear Regulatory Commission under provisions of 10CFR50.55 (e). Bechtel P..rer Corporation requested that Thermon Manuf acturing Company evaluate the situation to determine if Thermon should report to the Nuclear Regulatory Commission in accordance with 10CFR, Pa.t 21. Thermon Manufacturing Company has evaluated the situation and is submitting this report in accordance with 10CFR, Part 21.

The nature of the deficiency is that the terminal lugs utilized on the Resistance Temperature Detector (RTD) wire leads were of the wrong wire gauge size. Thermon Manufacturing ( ampany shipped a total of 475 RTD's Part Number RTD4946114P10038 (RTD-3) to the San Onofre Nucl. .r Generating Station, Units 2 and 3, San Clemente, California on Purchase Order Number V4138581. Approximately 300 of these RTD's were reported by Bechtel Power Corporation to have been inadvertently shipped with the terminal lugs attached. It is not the policy of Thermon Manufacturing Company to ship RTD's on this project with lugs attached. Resistance Temperature Detectors are terminated in the field by personnel other than Thermon Manufacturing Company.
