NATURE OF THE DEFECT OR FAILURE TO COMPLY AND THE SAFETY HAZARD WilICH IS CREATED OR COULD BE CREATED BY SUCH DEFECT OR FAILURE TO COMPLY:
Nature of Deviation:
A circumferential groove of varying depth up to 0.200 inch (18%)
was observed on the inside surface (I. D.) of journal approximately three inches above the attachment weld to the impeller.
Safety Hazard:
j Pump function is not safety related. Ilowever, pump pressure boundary is safety related.
Complete failure (severance) of the journal could result in major damage to pump internal components generation of loose parts; and potential breach of the primary pressure boundary to the e
component cooling water in the heat exchanger.
THE DATE ON WilICH THE INFORMATION OF SUCH DEFECT OR FAILURE TO COMPLY WAS OBTAINED:
Groove was first observed in reactor coolant pump on 27 May 1990 after inspection of the SONGS - 3 Unit.
Similar grooves, observed in reactor recirculation pumps, were evaluated and dispositioned in November 1989.
