Dear Mr. Keppler:

On June 27, 1980, Mr. S. J. Brewer of Public Service Company of Indiana, t Inc. , (PSI) notified your office of a potentially reportable item as required by 10 CFF 50.53(e). During the Construction Verification Program reinforcing stuel was inspected in accordance with SPP-2.

This inspection resulted in a nu=ber of nonconformances in the generic areas of clear cover, member thickness, size of reinforcing bar, number of bars and bar spacing.

In the interim report dated July 24, 1980, PSI committed to performing an engineering evaluation of these nonconformances and also described the manner in which any necessary repairs would be made to the affected structural elements. It was also-indicated that a more detailed description of these concerns and corrective actions would be available in Section 2.0 of the report on the Construction Verification Program which has since been submitted to your office.

In that report PSI determined, based upon engineering evaluations, that corrective measures would be necessary to meet the design requirements of a number of equipment pads which had dowels missing. Were these dowels to have remained missing, failure of the pads in question may have resulted if subjected to the extreme loadings which may occur during certain design basis events. Repair of these pads and other corrective measures which are being taken to prevent reoccurence are described in more detail in Section 2.0 of the report on the Constructica
