General: All Security Event Log entries from January 27, 2022 to April 27, 2023. Please provide any SELs that may take place after providing this information request to me when I arrive. Please provide the main CAP procedure.

1. Fitness For Duty Program (71130.08) (dated 01/1/2019)

A Corrective action report summaries that are any way associated with the Fitness for Duty program from February 01, 2021, to April 27, 2023. Please provide any FFD CRs that get written after April 27 to me while on-site.

B Audit reports concerning FFD since February 04, 2021.

C Exhaustive name list of FFD staff, either on-site or off-site, both contractors and proprietary employees.

D Provide the total number of personnel included in your FFD program.

E Policies or procedures describing your FFD program; to include employee expectations, Self-declaration, Fatigue assessment and description of the Employee Assistance Program, technique for random selection, collection procedures, MRO procedures, etc.

(if FFD Policy is not a standalone document please tell me which procedure it is in)

F The Behavioral Observation Program procedure, the Sanctions procedure, the Access Authorization procedure. (if BOP is not a standalone document please tell me which procedure it is in)

G Manufacturer and model of any EBT/ASD breath analysis equipment in use at your site.

Also any calibration equipment you may use.

H Name and address of all Laboratories used to support your FFD program.

Please be prepared to provide or discuss the following items upon my arrival onsite.

I Provide a list of any badged employees in a monitored Follow-up Program in 2021, 2022, and 2023.

J Demonstrate that the MRO and SAE have the required qualifications/certifications stated in 26.183 and 26.187 K Please provide a list of For Cause, Post Event tests completed in 2021, 2022, and 2023.

L Please provide a list of known subversion attempts completed in 2021, 2022 and 2023.

M Please provide a list of MRO Review cases completed for Wolf Creek under 26.185, in 2021, 2022, and 2023. Feel free to mark and secure the list in any fashion you are comfortable with. Additionally, you may retain the list until I specifically ask to review it during the inspection.

N Please provide a list of Determination of Fitness cases completed for Wolf Creek under 26.189, in 2021, 2022 and 2023. Feel free to mark and secure the list in any fashion you are comfortable with. Additionally, you may retain the list until I specifically ask to review it during the inspection.

O Please provide a list of positive alcohol tests completed in 2021, 2022, and 2023.
