Items may be provided on disc, in hard copy, Certrec, or a combination. NOTE: DO NOT forward any information on flash drives as current NRC procedures prohibit using this type of media unless issued by the NRC. Please properly package and mark SGI materials. If you have NON-SGI procedures or materials that you can send via email, feel free to do so to christopher.jewett@nrc.gov, although the use of CERTREC is encouraged. General: All Security Event Log entries since May 16, 2019. Security Program Procedure indices for all procedures, instructions, desk-top guides, standing orders, etc. Please state what revision of your physical security plan you are currently using.

Document Request List

1. Access Control (71130.02) (effective date 01/01/2019)

A. Corrective action report summaries that are any way associated with the Access Control program from May 16, 2019 to the date you provide this information request. Please provide any additional Access Control CRs that may be written after the delivery date while I am on site or through Certrec.

B. Any Access Control policy, procedure, desk-top guide, or any other type of lower tier instruction type document that you may use, such as: search and entry programs, VA alarm response, personnel and vehicle escort programs, access portal equipment testing, etc.

C. Current list of names who have UAA/UA to the PA. List of names who have UAA/UA to the VA. List of names who have been issued keys/codes to access control devices.

D. Any Lock and Key policy, procedure, desk-top guide, or any other type of lower tier instruction type document that you may use for the Lock & Key program.

E. Access Control Program related reviews, independent audits, or self-assessments since May 16, 2019.

2. Security Plan Changes (71130.09) (effective date 01/01/2020)

Please be prepared to provide the following items during the onsite inspection:

A. The latest version of the Physical Security Plan.

B. The security plan revision utilized May 16, 2019 (last baseline inspection).

C. All security plan changes that have been submitted to the NRC since May 16, 2019.

D. Procedures associated with plan changes since May 16, 2019.

E. Training material (i.e., lesson plans, briefing materials, computer-based training) developed to provide training to personnel responsible for implementing the plan change(s) since May 16, 2019.
