The inspection was conducted at the River Bend Station construction site by Mr. L. D. Gilbert during the period November 14-18, 1983, of

Response

VE-100 is a visual inspection procedure which requires verification of socket weld size, among other things. Reactor Controls Incorporated (RCI) Quality Assurance Instruction (QAI) 10-1 paragraph 4.4.6-C requires that socket welds be inspected for fillet size. This instruction has been in effect since the start of construction and socket weld

. sizes have been verified on 100% of the work.

Corrective Steps Which Have Been Taken and the Results Achieved Process Requirement Sheet (PRS) 02 has been revised. The new revision was issued to the jobsite on 11-28-83 and requires all socket welds to be inspected to VE-100. Each Inspector has now documented these inspections on the weld data sheets as part of the corrective action.

Corrective Steps Taken to Avoid Further Violations All Inspection personnel have been re-trained to the requireecnts of QAI 10-1 and PRS-02 Rev. 7 and the a documentation process. The char:Je to the PRS and provision for signoff will clarify and strengthen the requirement for the future. ,

A review of the other data sheets indicates that this is an isolated incident of the inspector signing the wrong data sheet pages. After this was inadvertently done, the Inspector used poor judgement in not correcting the error as soon as it was discovered.

Corrective Steps Which Have Been Taken and the Results Achieved The liquid penetrant examination weld check point on the weld data sheets for weld numbers I-2417-15, I-2017-15, I-0817-15, and . I-0417-15 were voided on 11-16-83. These welds were examined by liquid penetrant, found acceptable, and signed off 11-18-83.

Corrective Steps Taken to Avoid Further V,folations The Inspector has received a verbal reprimand for failure to follow procedure for signing QC Hold Points. He has also been re-trained in accordance with the RCI Quality Assurance Training Program.
