The violation involved inadequate design control. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1989),
the violation is. listed below:
Criterion V of Appendix B to 10 CFR Part 50, requires that activities ~
affecting quality, such as design changes, be controlled by. procedure.
Contrary-to the above, from June -1988, until August 1989, the procedures
.for.the control of design changes were inadequate in that they allowed an on-the-spot-change to alternate rod insertion system which substantially changed reset time without an evaluation.
