APPENDIX A NOTICE OF VIOLATION ,

Nebraska Public Power District Docket: 50-298 -

Cooper Nuclear Station License: DPR-46 e o i Dur ng an NRC inspection conducted on October 10 through November 20, 1993, a

  • l violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2,. .

Appendix C, the violation is listed below:  ;

Criterion V to Appendix B of 10 CFR 50 states, in part, that activities  :'

affecting quality shall be prescribed by documented procedures of a type appropriate to the circumstances and shall be accomplished in accordance with these procedures.

Maintenance Procedures 7.0.1.1, " Maintenance Work Requests - Work Item Tracking Form Initiation and Review," and 7.0.1.2, " Maintenance Work -

Request - MSR Generation and Review," were issued by the licensee to +

prescribe the licensee's expectations for activities affecting quality in the maintenance area. Step 4.12 of Procedure 7.0.1.2 describes the -

shift supervisor's general responsibilities for activities on his shift.

Step 4.3 of Procedure 7.0.1.1 requires the shift supervisor to review all. Work Item Tracking Forms for operability issues.

Contrary to the above, the shift supervisors in charge of the shifts on  !

November 18 and 19, 1993, failed to adequately review the work documents involved in the repair and testing of Fire Door H-305 and did not identify that this work resulted in a breach of the control room  !

isolation envelope.

This is a Severity Level IV violation (298/93026-02) (Supplement I) '

Pursuant to the provisions of 10 CFR 2.201, Nebraska Public Power District is hereby required to submit a written statement or explanation to the U.S.

Nuclear Regulatory Commission, ATTH: Document Control Desk, Washington, D.C. ,

20555 with a copy to the Regional Administrator, Region IV, 611 Ryan Plaza Drive, Suite 400, Arlington, Texas 76011, and a copy to the NRC Resident i Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).  !

This reply should be clearly marked as a " Reply to a Notice of Violation" and l should include for each violation: (1) the reason for the violation, or, if

  • contested, the basis for disputing the violation, (2) the corrective steps .;

that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. If an adequate reply is not received within the time specified.in this Notice, an order or a Demand for Information may be issued to show cause why the license should not be modified, suspended, or 9401040175 931229 PDR G
