I During an NRC inspection conducted April 16 through May 31, 1989, two violations of NRC requirements were identified. The first violation involved two examples of a failure to follow a safety procedure and an inoperable reactivity control system. The :;econd violation involved a failure to have Squib Valve A operable for a period of about 1 year in violation of plant technical specifications. In accordance with the " General Statement of Polic Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (y and1988),the violations are listed below:

A. Failure to Follow a Safety Procedure 10 CFR Part 50, Appendix B, Criterion V, states, in part, that

" Activities affecting quality shall be prescribed by documented instructions procedures or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."

CNS Procedure 0.9, " Equipment Clearance and Release Orders," Revision 7, dated February 23, 1989, specifies that the clearance order procedure provide a means of safely isolating equipment for repairs.

Contrary to the above, on May 18, 1989, the NRC inspector found Main Steam Line Drain Outboard Isolation Valve MS-M0-77 shut while danger-tagged open by Clearance Order 89-442. In additica, Clearance Order 89-827 utilized one tag to isolate seven valves for performance of maintenance.

This is a Severity Level IV violation. (Supplements)(298/8918-01)

B. Inoperable Reactivity Control System Technical Specifications 3.4.b.1 allows continued operation for only 7 days when a redundant component in the standby liquid control system is inoperabic.

Contrary to the above, the Squib Vahe A, a redundant component, was made inoperable on May 15, 1988, and remained inoperable until May of 1989.  !

The reactor operated in all Modes during that period.

2 Pursuant to the provisions of 10 CFR 2.201, Nebraska Public Power District'is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, l including for each violation: (1).the reason for the violation if admitted,

, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further_ violations, and (4)lthe date when full compliance will.be achieved. Where good cause is shown, consideration will be given to extending the response time.
