APPENDIX A NOTICE OF VIOLATION Nebraska Public Power District Docket:
50-298 Cooper Nuclear Station License:
DPR-46 During an NRC inspection conducted June 1-30, 1989, a violation of NRC i
requirements was identified. The violation involved inadequate design control.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is listed below:
Inadequate Design Control 10 CFR 50, Appendix B, Criterion III, requires that measures be established to assure that the design basis for structures, systems, and components are correctly translated into specifications, drawings, procedures, and instructions and that design changes, including field changes, shall be subject to design control measures commensurate with those applied to the original design.
Engineering Procedure 3.4, " Station Modifications," and Engineering Procedure 3.4.11. " Status Reports," requires procedure changes and training to be conduc,ted as applicable.
Contrary to the above, procedures for on-the-spot changes (0SC) do not require an evaluation to determine if procedure changes or training is required due to the OSC. Design Change 88-036 was altered by OSC No. 8 on April 20,1989, without training being conducted nor procedures affected by the OSC, such as System Operating Procedure 2.2.28, "Feedwater System,"
being modified to reflect the change.
This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Nebraska Public Power District is hereby required to, submit to this office, within 30 days 'of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved.
(3) the corrective steps which will be td en to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
