In compliance with 10CFR50 55 paragraph (e), we are sulmitting our complete Significant Deficiency Report No. 4 concerning the subject Core Spray Sparger.
Interim Reports (references a and c) were previously submitted to your office. We appreciate the final report submittal date extensions to my 15, 1974 provided by DRO I.

O O Significant Deficiency Final Report _-SDR #4 Unit No. 3 Core Spray Sparger Peach Bottom Atomic Power Station-Unit No. 3 AEC Construction Permit No. CPPR-38 Description of Deficiency An inspection of Peach Bottom Unit No. 3 Core Spray Sparger revealed certain unacceptable conditions which required the removal and rework of the sparger. The unacceptable conditions did not confom with the NSSS supplier requirements.
There were eighteen items found to be unacceptable by the NSSS supplier site Quality Control during the inspection. Conditions found which required corrective action include items concerning fabrication tack and fillet welds, are strikes, dimensioning in assembly of sparger components, and conditions of cleanliness and preservation.
Corrective Action The Core Spray sparger was removed from Unit No. 3 vessel, shipped to Reactor Controls, Inc. for rework to meet the NSSS supplier requirements, and reinstalled in Unit No. 3 vessel. Philadelphia Electric Company representatives observed the receipt inspection and reinstallation of the sparger assembly.
The documents associated with the repair and reinstallation of the sparger have been reviewed and approved by the NSSS supplier.
Philadelphia Electric Company Engineering has reviewed the documents and found them to be acceptable from a technical standpoint. The record package will be available at the site on June 15, 1974 if further review is required.
Safety Implications _
There are no safety implications following the rework of the sparger. The sparger, following the rework, conforms with the NSSS supplier requirements. Deviations from design requirements have been documented, reviewed and approved by the NSSS supplier...
