It has been datornised ttat vendor fabricated steel enbods designed to have full oenetration valds have less thn full penetration due to 4

improper veld ;oint preparation. S e cr. net mnber, identification, and location of the sumect ecbeds is the subject of en investigation, the results of which will be addressed in our final report.

2.

Corrective Action.

-2 B.

A hold has been placed on the installation of embeds requiring full penetration welda until satisfactory disposition of each embed is determined.

C.

To date one hundred twenty (120) vendor fabricated steel e= beds identified as nonconfo=ing have been returned to the vender for rework and reincpection to ascure compliance with purchase order require:cnts.

D.

To date twenty-nino (29) ir tallol c; beds with cartial penetration wolds whoro full penetration velds were called for have boon evaluated by 20chtcl Project Engincoring using actual loads and the "as built" usld configuration.

"ecause the actual locds are loca than the loada aseunod during preWhary-dosip, thoco twonty-nino nonconforming seldrents have been found adequnte by enalycis. lhe vendor supplied information on actual joint con 11g.u ation used and destructive exa=1 nation of three intricated embeds verified the vendor's data regarding weld joint configuration and weld penetration.

3 Evaluation of 39fatv hulications.

There will be no adverse safety icplications ainee the rework or engineering evaluation will ensure that embeds fabricated to date are structurally adequate.

4.

Schedule for Submitting Finc.1 Pnm rt Investigations are continuing to further identify the scope of this deficiency. Because of the quantities, different types and various locations, we expect that this investigatics will take considerable ti=e and effort.

L'e will e.dvise you in sixty days of our progress toward submittal of a final report.

Sincerely,
