Dear Dr. Murley:
The nature of the casualty on the subject generator was a failure of the inner polar connector between the damper bar circuit on the generator main field.
At the present time the cause of this failure has not been defined and further investigation is necessary.
There are four sites that utilize generators of this type. They are as follows:
- 1. Vermont Yankee @ Vernon Station, Vernon, VI - 2 units
- 2. Philadelphia Electric, Peach Bottom 2 & 3 - 4 units
- 3. Baltimore Gas & Electric, Calvert Cliffs 1 & 2 - 3 units
- 4. Metropolitan Edison, Three Mile Island - 2 units Locations 2 and 4 were notified by others of the potentir 1 problem and have been in contact with Louis Allis for additional information. Location 1 above was notified by telephone on May 29, 1985 by the writer. The person contacted was Mr. Don Dyer, Operations Quality Coordinator.
At the present time Louis Allis engineering is reviewing the reports from the field in an effort to determine the cause of failure and will be forwarding that information to Fairbanks Morse / Colt Industries for their corrective action.
