Dear Mr. Keppler:

On April 9, 1979, Mr. F. R. Hodges of Public Service Company of Indiana, Inc. (PCI) notified your office of a potentially reportable incident involving the contain=ent liner on Unit 1. On May 9,1979, PSI transmitted to your office an interim report describing the progress on this incident to date and plans for its resolution. The following report resolves that item and constitutes a final report under the regulation.

On April 6, 1979, two construction cranes collided, with the counterweight of a tower crane striking the jib of a 4100 crane, which then fell and struck the containment liner. Damage to the linar and reinforcing occurred in two places. At elevation 525', one vertical angle stiffener and the circular C8 ring stiffener were da= aged. At approximate elevation 464', three #11 vertical and six #11 horizontal reinforcing bars were damaged. If this damage had remained uncorrected it could have adversely affected the safety of operatic.s of the plant.

Although, any breach of the leak tightness of the liner would have been detected in the Integrated Leak Rate Test, the liner anchorage system had been damaged and this damage may have manifested itself as a liner failure under accident conditions.

As a result of this incident the damaged reinforcing bars were replaced and the vertical angle stiffner and circular ring stiffener were repaired. Additionally, a dent in the post-tansioning shcathing was cut out and the sheathing rejoined with a standard coupler.

These crane accidents are not to our liking and we do everything in our power to prevent any type accident, to say the least of crane accidents. N-:G has once  ;

again advised all shift operations as to where to dog j off the 4 600 where it cannot come into contact with a the Tower Crane, also we recu 2sted the crerators Q assure themselves that brake is 3ocked into position.

l At the present time we are anticipating the removal of the Tower Crane very soon (apprcximately 2-3 weeks) .

We feel this will totally eliminate any additional crane accidents.
