Response to Inspection Report 50-482/78-12

Dear Mr. Seidle:

In our initial reply to your letter of November 29, 1978, which trans-mitted Inspection Report 78-12, we requested that additional time be allowed to fully respond to the finding regarding the welding of the diesel fuel oil tanks. The response to this finding follows and is made in three parts:

a) Corrective steps which have been taken and the results achieved, b) Corrective steps which will be taken to avoid further non-compliance, and c) The dates when full compliance will be achieved.

1. Finding The RECO procedures controlling welding do not specify require-ments for cleanliness and protection of welding surfaces. The IE Inspector observed on November 8,1978, that mill scale was not being removed from the base metal prior to welding of the emergency fuel oil tanks.

a) The RECO procedures controlling manual and automatic welding have been revised to incorporate the following supplement 7906140 / 7R Q 201 N. Market - Wichita, Kansas - Mail Address: P.O. Box 208 / Wichita, Kansas 67201 - Telephone: Area Code (316) 2641111

Mr. W.C. Seidle April 25, 1979 regarding cleanliness and protection of welding surfaces:

"The method used to prepare the base metal shall leave the weld preparation with reasonably smooth surfaces.

The surfaces for welding shall be free of scale, rust, oil, grease and other foreign material. The work shall be protected from foreign contamination and from rain, snow and wind during welding. Welding shall not be -

performed on wet surfaces." -

In addition, the statement, " peening is not permitted", has been incorporated into these welding procedures and the procedures have been submitted to the Architect / Engineer for review and approval.

Daniel Q.C. performed visual inspection on suspected seam No. 20 (Job 238) and found it acceptable. This examination included inspection for lack of fusion and overlap. In addition five (5) randoc radiographs were taken on seam No. 20 and two (2) radio-graphs were taken on seam No.19 (Job 238) at the nost suspect area as determined by visual inspection. Results of radiogrnyhs showed satisfactory welds. RECO also reviewed all initial fabrication radiographs for Jobs 233 and 238 and found them acceptable.

We believe the above investigations and actions are indicative of a satisfactory quality at the welds on Jobs 233 and 238.

b) Reiterating our initial response of December 28, 1978, Daniel Q.C. will verify that NECO welding procedures contain require-ments for cleanliness protection of the welding surface prior to their resuming welding operations. In addition, Daniel Q.C.

will perform surveillance inspections to assure that RECO implements these requirements for any additional carbon steel tank fabrication which may be performed. The revision of procedures, together with regular surveillance, should avoid further non-compliance.

c) procedural changes have been completed and submitted to the Architect / Engineer for review and approval. Surveillance inspections will be initiated when RECO returns to the Wolf Creek site to continue welding operations.

Full compliance will be achieved by May 28, 1979.
