example, the corrective action did not appear to address the potential for other safety-related equipment to be similarly effected.

Response

During a review of the ESF *tatus panel, no other instances were identified where the mispositioning of ESF equipment contro11erv would not be

!. annunciated on the ESF status panel. Annunciation on the ESP status panel l of equipment controller mispositioning clearly identifies to the operators the status of the equipment, allowing appropriate actions to be determined l and taken. I Even though mispositioning of the speed controller (FC HIK-313A) for the L

, TDAFWP governor control valve le not directly annunciated on the ESF status l

panel, there is an alarm window on main control board 128-E, 'TD AFP SPD HILO", which becomes active when the TDAFWP mechanical trip and throttle L

valve, FC HV-312 is opened. This window will alert the operator if the pump is started and the speed setting is not correct. The alarm response  !

procedure for this window, ALR 00-128E, 'TD AFP SPD HILO' requires that the operator immediately ensure that the TDAFWP governor control valve speed controller (FC HIK-313A) is at its normal output setting. Although this '

alarm function.cannot prevent mispositioning of the speed controller _it does provide the ability to promptly return the speed controller to the appropriate setting if the TDAFWP is started.  ;

In addition, a change has been made to ADH 02-005, ' Reactor Operators

-Qualifications and Responsibilities' clearly stating the need for the Reactor Operator to be cognizant of plant conditions. This change was made to address an NRC concern regarding the Reactor Operator's awarenees of plant conditions.
