REFERENCE:

Peach Bottom Units 2 and 3 i Technical Specifications 3.14.A.8.b This special report, involving the inoperability of a spray nozzle in the Standby Gas Treatment charcoal filter fire suppression system, is being submitted pursuant to the referenced Technical Specification.

. Ids g fication of Occurrence l

On January 3, 1990, with Unit 2 operating at 100% power and Unit 3 at 65%

L ' power the common plant Standby Gas Treatment System (SBGT) "A" train fire

-protection deluge system was declared inoperable. During performance of a surveillance test one of thirty-six deluge system spray nozzles protecting the 1

~SBGT:"A" train charcoal filter wa3 discovered to have been replaced with a v -threaded pipe plug. The deluge system was not restored to an operable-status L until January 22, 1990. This condition is reportable pursuant to the L -referenced Technical Specification.

Cause of Occurrence

! The. nozzle was not plugged at the time of the previous system air flow

test and' visual inspection, performed July 6, 1988 in accordance with 1 Technical. Specification 4.14.A.7.b and c. A review of maintenance records did not identify any work activities associated with these nozzles after that Ltime. Time necessary for procurement of a replacement nozzle resulted in x

exceeding the referenced Technical Specification 14 aay restoration period.

,7 Actions Taken A fire watch patrol was immediately established to inspect the affected area at least once
per shift. On January 22, 1990, the nozzle was replaced and the system returned to service.- Periodic surveillance tests of other fire suppression systems have not revealed any other plugged nozzles.
