Improper testing following installation of a jumper.

l l Analysis of Occurrence:

1 A jumper was installed in the control circuit on 5/20/79 during an investigation of the inability of the Unit 2 recirculation system sample isolation valve A0-2-2-40 to open in response to a manual signal following a modification to assure electrical separation. Because the jumper restored manual eperat ton and because the individual doing the work be11oved that the

.tumper only restored :ircuit continuity, no testing of the isolation cep-hility of the valve was done. The juusper was desir logged in the jissper tog per procedure. On 12/7/79, during preparation for testEng of another modification this jumper was recognized to inhibit automatic isolation of that waive. However, the inner isolation valve was not affected and would have automatically isolated upon roquest. Becauso

  • the innor isolation rutvo would have isolated and the outer isolation valvo could be operated manually, the line could have been isolated if necessary.

Corrective Aetion: A -

The jtamper was removed upon discovery. Both valve 6 were tested for 4

manual and automatic isolation and were found to I a ftally operable.

Am imunediate review was made of jumper los entrie- concoming RP5, p
