During an investigation of another matter. it was discovered the diesel generstar eerdox system vepor pilot valve vem closed.
The vapor pilot valve must be open to provide pressure to open the diesel room C02 deluga valves.
Asserent Cause of occurrences Failure to properly clear a safety permit on 7/3/80.
Anal:rvis of occurrence:
The vapor pilot valve was blocked shut on a safety po s it on 7/3/80 to allow required functional test to be performed on the diesel generator ro m host detectore.
This prevente actual discharge of CO2 into the diseal room during the perfomance of the test. When the safety pe mit was cleared, the valve was apparently imeerrectly left in the closed position.
Corrective Actica:
Me vapor pilot valve was locked open.
The system drawings (PEID). systes em list and adaimistrative procedure A-8 Iocked Yalve List are being revised to include the vepor pilot valve as a lockma open valve.
