5 Emproper instastation of ancher botts.
Analysis of Occurrence:
i pering the inspection of hanger number 32-GR-541 two belts failed due to shell projecties from the estl of greater than I/R inch.
4 Failure of this support anchor.may not have remmited in fafIure of the. supported piping system during a seismic event since alt four bolts in the sucher still meet the torque requirements.
Nomever, even if this pip 5mg system wantd have failed, only two containment.cet ting subsystems wonto be affected and both LFCI subsystems would still remain operable.
