Ayyarent Cause: 5 Emproper instastation of ancher botts. ,

Analysis of Occurrence: i pering the inspection of hanger number 32-GR-541 two belts failed due to shell projecties from the estl of greater than I/R inch.

4 Failure of this support anchor.may not have remmited in fafIure of the. supported piping system during a seismic event since alt four bolts in the sucher still meet the torque requirements.

Nomever, even if this pip 5mg system wantd have failed, only two  !

containment.cet ting subsystems wonto be affected and both LFCI subsystems would still remain operable.
