Tallure to maintain secondary contairment integrity during survenent of irradiated fuel within the spend risol pool.

Conditions Prior to Occurrence:

Unit 2 shutdown: all fuel renoced fYocs the reactor.

Apparent Cause of Occurrence:

Failure to adequately specify required conditions under which modification to secondary conteirunent boundary could be perfor: sed.

Analysis of Occurrence:

On %e riorning of May 2,1954), an irradiated fuel bundle was relocated in the spent ruel pool for the purpose of lead test assembly inspection.

A procedure which delineates the conditions tander which ressavat of these boot seats is permitted will be prepared so thin type of error will he prevented in the future.
