Bott of Public Service Company of Indiana, Inc., (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e). During installation of temporary non-safety related pipe, concrete expansion anchors used for support of pipe hangers were placed in a safety related concrete structure (Auxiliary Building) without approved procedures.

PSI personnel have surveyed the anchor installations to determine what, if any, damage may have occured. The results of this survey (Attachment 1) were submitted to PSI's architect-engineer for evaluation.

The architect-engineer evaluated this information from a design standpoint and concluded that the installation of the subject concrete expansion anchors would not impair the safety of the structure involved.

To prevent recurrence of this problem in the future, PSI has sent letters to each site contractor indicating that no attachments are to be made to safety related structures without PSI approved procedures unless such attachments are shown on approved drawings.

As those anchor installations did not impair the safety of the structure involved, this item would not have affected adversely the safety of operatior.s of the plant and therefore is not reportable under 10 CFR 50.55(e).
