! dation in an attempt to minimize S/G tube denting. .

Test with PORV Operability Not Required) when he accidently opened
- the wrong switch in the protection and control racks. This l resulted in a steam flow / feed flow mismatch and low Channel I S/G level, causing a turbine trip / reactor trip. The periodic test s
was re. vised to clarify switch operation and the instrument technician was reinstructed.
80-16 (A) 6/14/80 Time: 1200. Decrease reactor power to 40% to al bw contain-ment entry to repair A S/G LT-1474 root valve. The root i..ve had a body to bonnet leak resulting in an erroneous level signal being sent to the protection circuit. The valve was firminited, leak stopped, and power was again escalated to 100%.
80-17'(A) 6/18/80 Time: 0905. An instrument technician was working in process rack no. 6. When.he completed his work he closed the rack door which jarred a loose jumper on a process card. This resulted in a steam flow / feed flow mismatch and A S/G low level causing a turbine trip /
reactor trip ~. The, card with the loose jumper was replaced. A random inspection of process cards will be performed to identify any other loose jumpers.
80-18 (H) 6/26/80 Time: 1422. . An instrument technician was calibrating N43 and a spurious spike occurred on one of the other two channels. This
, resulted in a overtemperature AT reactor trip _. Subsequent to this

when a broken air line was discovered on one of the actuating ,
_ cylinders for B Main' Steam Trip ~ Valve. ' The broken air line was '
replaced and the generator placed on the 'line. Power was escalated to 100%. -
