SB00ED STEAN CEEERATOR TURE IESERVICE INSPECTICE RESULTS This report is being submitted pursuant to Technical Specification (T/S) l 4.4.5.5 b and 6.9.2 to report the results of the second steam generator tube l inservice inspection. .

The second steam generator tube inservice inspection of steam generators '

(S/G) was completed by Westinghouse Electric Corporation on November 18, 1988, during the Refueling III outage. This inspection included all four steam generators, 'A', 'B', 'C', and 'D'. It was the first inservice- 1 inspection of S/G's 'A' and 'D' and the second for S/G's 'B' and 'C'.

A total of 11,889 tubes were inspected (52.8%), and a total of 22 tubes were plugged as discussed in Special Report 88-003, which was transmitted by ,

letter WM 88-0311, dated November 29, 1988. Included in this inspection j effort were three tubes which had been previously identified as having '

indications requiring re-inspection during this inservice inspection.

effort.. As discussed in Special Report 86-009, these tubes are R55-C62 and  :

R47-C99 in S/G 'B' and R36-C110 in S/G 'C'. The inspection results of S/G

  • B" tube R47-C99 are tabulated in section 5 of this report.- During the f inspection of tubes R55-C62 in S/G 'B' and R36-C110 in S/G 'C' no reportable .;

indications were found.

In addition, this inspection effort encompassed five tubes with reportable indications during the preservice inspection. Thece five tubes are R23-C22 in S/G 'B' and R2-C19 R21-C64, R56-C77, and R41-C87 in S/C 'C#. Both the preservice inspection and first inservice inspection results for these tubes are delineated in Special Report 86-009. During the 1988 inservice inspection, no reportable indications were identified for any of these tubes. ',

Reviews of the inservice inspection data to date have been unsuccessful in  ;

developing an explanation of the conditions leading to the disproportionate

'

L number of indications of tube wall thickness loss identified in S/G 'D'.
