Title:

PREPARATION AND APPROVAL OF DISCIPLINE SPECIFIC RESULTS REPORTS AND DAP COLLECTIVE EVALUATION REPORT PURPOSE This addendum changes the definition of the section's scope.

SCOPE Revise Section 6.0 of Attachment C of DAP-9, Rev.1 (pC-2) to read:

6.0 ROOT CAUSES AND GENERIC IMPLICATIONS "This section describes the root cause and generic implicottons methodology used by the Design Adequacy Program and h) discusses the conclusions reached by its application.

This V

section also discusses root causes potentially attributable to programmatic elements of the design process that have been referred to the OA/OC TRL. Information received from the OA/QC RTL regarding quality program weaknesses offecting design and his recommendations for improvement for both ongoing and future programs shall be discussed. This section will supplement these recommendations with appropriate discussions of engineering work processes, management of these processes, and the effectiveness of implementation of the design OA program."

INSTRUCTION This addendum is ef fective upon approval and shall remain in effect until opproval of the next revision of DAP-9.

This addendum shall be securely attached to each controlled copy of DAP-9.
