1.0 PURPOSE This procedure defines the methods to be used to occomplish, maintain, and document the training and qualifications of personnel as required by the CPRT Progrom Plan.

2.0 SCOPE This procedure opplies to the training and qualification of personnel assigned to the conduct of the Design Adequocy Program and to the resolution of TRT issues in the Civil / Structural, Mechanical and Miscellaneous Areas (CSM). This i

procedure supersedes Work Instruction CSM-1; however, training records compiled under CSM-l shall be deemed to meet the requirements of this procedure and do not need to be revised.

3.0 DEFINITIONS AND RESPONSIBILITIES 3.1 Definitions 3.1.1 Third Porty Personnel j Non-CPSES project personnel assigned by the Review Team Leader to manage or conduct the reviews and evoluotions performed under the DAPs or the CSM l

TRAINING AND QUALIFICATIONS Revision: 2 1.l.3 Support Personnel Administrative and clerical personnel assigned to support the DAP and the CSM ISAPs.

3.1.4 Third Party Inspection and Test Personnel i

Non-CPSES project personnel assigned to the DAP and the CSM ISAPs who perform inspections and tests within the scope of the CPRT Policy on Testing and Inspection Personnel used in Third Party Verification Activities.

3.2 Responsibilities l

3.2.1 Review Team Leader

! o Approves of the assignment of Third Party personnel to the CPRT program through the DAP and TRT (CSM) personnel roster.

3.2.2 Design Adequacy Program Manager (DAP Manager) o Designates a Training Coordinator for the DAP.

o Identifies to the DAP Training Coordinator the individuals assigned to DAP for whom a training record will be

, maintained and their training requirements. This notifi-cation may be via written or oral notice. The DAP Manager may delegate this activity to the Discipline c Coordinator. This notification shall be documented in the training record on Attochment A.

o Notifies the Training Coordinator when revisions of docu-ments have been issued which affect personnel assigned to the DAP. He specifies the extent to which retraining is required for each procedure as a result, based on the nature and extent of the procedure revision.

TRAINING AND OUALIFICATIONS Revision: 2 1-3.2.3 TRT issues Monoger o Designates a Training Coordinator for the CSM/TRT issues, o identifies to the TRT Training Coordinator the individuals assigned to CSM/TRT issues for whom o training record will be maintained and the training requirements. This notification may be via written or oral notice. This notification shall be documented in the training record on Attochment A.

o Notifies the Training Coordinator when revisions of docu-ments have been issued which offect personnel assigned to resolution of CSM/TRT issues. He specifies the extent to which retraining is required for each procedure, based on the nature and extent of the procedure revision.

3.2.4 Training Coordinators o Establish a DAP training file, to be maintained in accord-once with DAP I4, which documents that the training requirements identified by the DAP Monoger have been met, o Establish a CSM/TRT training file, which documents that the training requirements identified by the CSM/TRT Monoger have been met.

TRAINING AND OUALIFICATIONS Revision: 2 4.0 INSTRUCTION 4.1 Approval of DAP and CSM Third-Porty Personnel The DAP and CSM Review Team Leader shall maintain a roster of all third-party personnel opproved by him. He may delegate responsibility for maintenance of the roster to the DAP Monoger who may maintain the roster for CSM personnel as well as DAP personnel. The roster shall be in the format of Attachment F.

For third-party personnel assigned to the project prior to December 15,1985, the DAP Manager shall develop the initial DAP and CSM roster by January 10,1986.

The initial DAP roster shall be submitted to the RTL together with resumes and objectivity forms for review and opproval. The RTL shall indicate his approval of those personnel by memo to the DAP Monoger and to the TRT issues Monoger.

In his memorandum, the RTL shall note any limitotions on the assignments of the personnel because of either limitations of their qualifications or information contained in their objectivity forms.

For third-party personnel assigned to the project offer December 14,1985, the person recommending on Individual for assignment to DAP or CSM octivities shall submit to the RTL the following information through on Action Memo:

For administrative and clerical personnel, only the individual's nome and position need to be submitted from the above list. The Action Memo shall also identify I

the person responsible for specification of the individuol's training requirements.

After the review, the RTL shall advise the originator of the request by memo whether the individual is accepted and whether there are any conditions or

limitations on the assignment of the individual. The RTL shall provide copies of his response to the DAP Monoger, the TRT Issues Monoger, and the oppropriate l

) Training Coordinator. The RTL (or his designee) shall enter the individual's name on the roster offer opproval.

The above Action Memo is not required for personnel assigned prior to December 15, 1985. The initial roster developed by the DAP Manager shall note those individuals assigned by to this date by entering the phrase " Initial Roster" in the

" Recommended By" column.

4.2 Training Records The personnel qualification and training records needed to meet the require-ments of the CPRT Program Plan ore:

o Resume covering education and work experience for Third Party personnel with technical responsibilities.

o CPRT Objectivity Questionnaire for Third Party personnel (Attachment D).

o A record for Third Party inspection and Test personnel (other than those assigned through the OA/OC Review Team Leader) that confirms that their qualification re-cords have been reviewed and are acceptoble (Attach-ment E).

Alternative forms may be used in lieu of those ottoched to this procedure, provided that the alternative form contains information equivalent to the standard form.

The Training Coordinator shall establish and maintain training and qualification files in accordance with DAP-14 and CSM/TRT instructions as appropriate.

4.3 Training Requirements The DAP Manager and CSM/TRT Manager shall identify to the oppropriate Training Coordinator the training requirements, required completion dates of .

training (per Section 4.4), and qualification records needed for each individual assigned to their programs. He shall also indicate where o resume or Objectivity Questionnaire is not required because of the scope of the individual's respon-sibility. (Resumes and Objectivity Questionnaires are not required for support personnel).

As a minimum, third party technical personnel shall be trained on the provisions of the CPRT Program Plon, the Design Adequacy Plan (Appendix A), the Design Adequacy Quality Assurance Program, and DAP Procedures applicable to their scope of work, as well as ISAPs/DSAPs on which they are working. Support personnel shall receive formal training to the extent necessary to perform their

.- functions. Any combination of lecture, reading assignments, and "on-the-job" training may be used to meet the training requirements. Consultants who are recognized experts in specific technical orcos may be exempted from this requirement provided their role in the implementation of on ISAP or DSAP is limited to providing consultation in their porticular creo of expertise.

TRAINING AND OUALIFICATIONS Revision: 2 The DAP Monoger and the CSM/TRT issues Monoger may delegate the develop-ment of specific training requirements (e.g., specification of particular DAP procedures for which the people should be trained) to others.

4.4 Notification of Training and Completion The Training Coordinators shall notify each identified individual of their required reading assignments (and required completion date) and obtain written confirmo-tion that all required training has been completed. Any reasonable period for completion of training may be allowed provided the following requirements are met. Personnel shall complete their training relative to their specific scope of work prior to performing that work; however, this requirement does not opply to work performed during a period of "on-the-job" training. The period of "on.the ,

Job" training shall not exceed 30 days. An individual's initial training assign-ments (including "on the job" training) shall be complete prior to Discipline Coordinator opproval of any work product developed by the individual. Personnel shall complete their general training (e.g., reading of the applicable DSAP) within 30 days of their assignment to the DAP or CSM-TRT octivities.

Exceptions to these training completion requirements may be authorized by the RTL (or his designee) if the person requesting the exception proposes actions deemed by the RTL (or his designee) to be odequate to ensure that delay in the completion of the training will not offect DAP work products. The Training Coordinators shall notify their respective monogers of problems in completing the identified training requirements within the time established by the monoger.

l 4.5 Training of Third Party Inspectors and Test Personnel The Training Coordinator for CSM/TRT lssues shall obtain a list of Third Party inspection and Test personnel (other than those assigned through the OA/QC RTL) assigned to CSM/TRT Issues from each Issue Coordinator and submit the  !

4.6 Re-Training of Third Party Personnel For revisions of DAP procedures approved after December 14, 1985, the DAP Monoger shall issue a memorandum to the Training Coordinators, Discipline coordinators, and others, as appropriate, that specifies the extent to which re-training of personnel is required. Revisions deemed by the DAP Manager to be minor or administrative in nature shall not requi;e that all DAP personnel originally trained in the procedure be re-trained in the revised procedure. Re-training of personnel originally trained in a procedure shall not be required if the person will not be using the revised procedure.

O For revisions prior to December 15, 1985, the DAP Manager shall determine whether any of the prior revisions to DAP procedures may be considered to be minor revisions such that re-troining is not required for personnel trained to o previous revision.

, 4.7 Ouolifications of DAP Personnel l

The action of acceptance of a proposed individual by the RTL indicates his acceptance of the individual's objectivity and his (or her) knowledge and technical experience for assignment to the DAP or TRT-CSM issues, as oppropriate. For DAP personnel, on additional qualification requirement has

o Each Discipline Coordinator shall determine, using the criteria below, which design areas within his discipline are unique to nuclear power plants.

o Each Discipline Coordinator (or his designee) shall advise by memo personnel assigned as reviewers in his discipline of the areas determined to be unique to nuclear power.

o The memo shall also contain the following statement:

"The DAP is committed to performing its work with qualified personnel. Personnel have been selected for participation in the DAP by considering their technical expertise and objectivity. To provide additional confi-dence that the experience of our staff is oppropriate for O their review tasks, we have also made two specific commitments. First, for design areas unique to nuclear power plants,, reviewers shall only review design areas which they have personally performed previously on a commercial nuclear power plant.

Second, for all other areas, reviewers shall only review design oreos where they have had applicable experience (i.e., need not be commercial nuclear power plants). If you are assigned to work in on area for which you believe you do not meet either of these requirements, notify your supervisor or Discipline Coordinator immediately. Please sign this memo and return it to your Discipline .

Coordinator to acknowledge that you have been advised of these requirements, have had and opportunity to obtain clarification, and have been provided with a list of design areas unique to the nuclear power plants."

o The Discipline Coordinator (or his designee) may provide other guidance and supplementary information as deemed oppropriate.

TRAINING AND QUALIFICATIONS Revision: 2 For the purpose of this procedure, the following factors shall be considered in the determination of whether o design area is unique to nuclear power plants:

1. Specialized codes and standards unique to nuclear power plants (e.g., ASME Section lil)
2. Regulatory requirements for nuclear power plants that are not specifically documented on criteria lists developed in accordance with DAP-l such that commer-cial nuclear experience is necessary to review the creo adequately.
3. Other design considerations unique to nuclear power plants that are not specifically documented on criteria lists developed in accordance with DAP-l.

These foetors shall be opplied to design arcos determined by the Discipline Coordinator to be oppropriate in scope for determination of whether the area is unique to nuclear power plants. For example, the design review topics described in the Discipline Specific Action Plans (CPRT Program Plon, Appendix C) may be used; however, the designation of areas considered ng to be unique to nuclear power plants shall not be so brood as to foil to identify those arcos that should be considered unique to nuclear power plants.

5.0 DOCUMENTATION The Training Coordinators (located in Bethesda for DAP and at the CoSES site for CSM ISAPs) shall maintain the following records in the DAr) files in accordance with DAP-14 and CSM/TRT instructions as oppropriate:

o Individual Training Records (Attachment A) o individual Resumes o Record of Assigned Reading (Attachment B) o Record of Training Session (Attachment C) o Objectivity Questionnaire (Attachment D) o Record of Third oorty inspection and Test Personnel Qualification (Attochment E).

o Documentation of compliance with Section 4.7 of this procedure.

TERA Corporation's CPRT octivities are being performed by TENERA Corporo-tion, the Company's nuclear services division. In fiscal year 1982, this division

! performed services for TUGCO's Operations Department with associated revenues of less than I percent of the nuclear division's total annual revenue. This contract was TEtERA's only contract with TUGCO or its offiliates with the exception of

, the existing contract associated with the Company's CPRT role.

TERA Corporation, the parent company, has had prior contracts with TUGCO and its offiliates in the areas of automated records management, computer and environmental services. For each of th last five fiscal years these revenues have represented less than 10 percent of TERA's total revenues. TERA Corporation has not been involved in any nuclear safety-related work octivities for Texas Utilities.
