DAP Discipline-Specific Results Reports and the DAP Collective Evoluotion Report. This procedure also defines the review and approval process of these reports within the DAP.

I 2.0 SCOPE This procedure opplies to the preparation and opproval of Discipline Specific Results Reports and the DAP Collective Evoluotion Report. This procedure does not oddress the preparation of supporting documentation (such as on engineering

evoluotion that is covered by DAP-8).

, 3.0 DEFINITIONS AND RESPONSIBILITIES 3.1 Definitions i

! 3.1.1 Discipline Specific Results Report i

A Discipline Specific Results Report is that report required by the CPRT Program Plan to assess overall DAP conclusions regarding the odequacy of design within the subject discipline.

l t

3.1.2 DAP Collective Evoluotion Report <

The DAP Collective Evoluotion Report is that report required by the CPRT Program Plan to assess in a collective fashion the adequacy of the design of Comanche Peak.

PREPARATION AND APPROVAL OF DISCIPLINE SPECIFIC RESULTS Revision: 1 REPORTS AND DAP COLLECTIVE EVALUATION REPORT otherwise approved by the DAP Monoger. When he is satisfied with the content of the Results Report, the Discipline Coordinator shall sign the report cover sheet (Attachment B) and forward the report to the DAP Manager.

The DAP Manager shall review the Discipline-Specific Results Reports forward-ed by the Discipline Coordinators. His review shall consider whether the results report meets the requirements of this procedure, the CPRT Program Plan, and the Design Adequacy Plan. He shall otso determine whether the scope of the report is consistent with the scope defined for the discipline by the process described in DAP-3. Furthermore, he shall evoluote whether the report is internally consistent and consistent with supporting documentation such as s checklists and erygineering evaluations. The DAP Manager may delegate specific review tasks provided that such reviewers do not review portions of the report for which they were responsible and do not review portions of the report that are supported by review activities (e.g., evoluotions, checklists) for which they were responsible. When the DAP Manager is satisfied with the report, he shall sign

! the cover sheet and forward the report to the Review Team Leader.

l The Review Team Leader shall discuss the content of the report and its conclusions with the Design Adequacy Manager and the Discipline Coordinator.

l He shall determine the extent and method of any further reviews. The Review l

Team Leader, DAP Manager, and the Discipline Coordinator shall resolve any comments on the report. The Review Team Leader shall then sign the report cover sheet to indicate his approval. The Review Team Leader shall forward the report to the CPRT SRT and resolve any SRT comments. Changes to the Results Report as a resolution of SRT comments shall be submitted to the SRT os a b revised Results Report and the revision number identified. Specific actions taken as a result of SRT comments shall be documented by the Review Team Leader.

The DAP Manager shall prepare (or cause to be prepared) o DAP Collective Evoluotion that shall be consistent with the format and content of Attachment C unless otherwise approved by the Review Team Leoder. Upon completion of the report, the DAP Manager shall sign the cover sheet (Attachment D) and forward the report to the Review Team Leader.

The Review Team Leader shall review (or cause to be reviewed) the DAP Collective Evoluotion. This review shall determine whether the Collective Evoluotion is consistent with the Discipline Specific Results Reports and other i information developed by the DAP. The Review Team Leader and the DAP

, Manager shall resolve any comments. When he is satisfied with the report, the l Review Team Leader shall sign the cover sheet to indicate his approval. The Review Team Leader shall forward the report to the CPRT SRT and resolve any SRT comments. Specific octions taken as a result of SRT comments shall be documented by the Review Team Leoder.

5.0 DOCUMENTATION 5.1 Identification After opproval has been obtained, each Discipline-Specific Results Report shall be assigned a control identification number by the DAP Manager. The identification number shall be of the following format:

SUMMARY

AND CONCLUSIONS Include purpose, scope, results, and conclusions in summary form.

2.0 SCOPE This section describes the scope of the report and, therefore, the scope of the design to which the conclusions are applicable. The interfaces between the discipline covered by this report and other disciplines should be addressed.

3.0 EXTERNAL SOURCE ISSUES 3.1 Description This section should describe the external issues covered by this resuhs report and relate the issues to the action plan (s) that resolve O them.

3.2 Methodology and Results This section describes the methodology applied to the resolution of the external source issues. It should describe the activities under-token to evoluote the issue, the classification of the issue (observo-tion, deviation, or deficiency), and the method and extent of third party activities. It should also summarize the results and bases of the safety significance evoluotions applicable to the discipline. The /A root cause (if known) and generic implications of external source a issues determined to be deficiencies should also be discussed. Where generic implications or trending required expansion of the scope or opproach from that presented in the action plan, o detailed descrip-tion should be provided. This section may be organized in a manner deemed oppropriate by the discipline coordinator. For example, each issue could be treated as a separate subsection within 3.2 and a discussion prepared for each. Alternatively, the section may be arranged by source of the issue (Cygno, CASE, NRC, etc.) or by grouping related technical issues.

CPRT DESIGN ADEQUACY PROGRAM O DISCIPLIE-SPECIFIC RESULTS REPORT FORMAT (Continued) 3.3 Conclusions This section presents the conclusions found by the third party relative to external source issues only.

4.0 SELF-INITIATED REVIEW 4.1 Scope This section discusses the scope of the self-initiated review and the i

basis for that scope. Specifically, it should address the evolution of i that scope from that presented in Rev. 2 of the CPRT program plan, through the changes resulting from Phase 3 of the scope development process and from implementation of the action plan, to the final (Phase 4) scope.

4.2 Methodoiogy This section describes the methodology applicable to the self-initi-ated review. It should concentrate on how the general requirements O of the program plan were opplied to the specific discipline covered by the report. It should also summarize the results and bases of the safety significance evoluotions applicable to the discipline, it is not A

Ih necessary to repeat information contained in the action plans; how-ever, enough information must be provided to allow a reviewer (e.g.,

NRC or public) to obtain on understanding of the work that was done.

Where oppropriate, lists of documents reviewed, references used, etc.

should be provided.

4.3 Findinas 4.3.1 Deficiencies List all deficiencies found together with a description of the deficiency, the corrective action, root cause (if known),

generic implications, and other relevant dato.

i. 4.3.2 Deviations with Adverse Trends Discuss those deviations found as a result of action plan implementation that were analyzed to be part of a trend.

Specifically, this section should oddress the nature of the deviations, the bases for reoching a conclusion that a trend exists, on onolysis of the trend, and the corrective action taken.

1 O TN-85-6262/9 A-2

O DESIGN ADEQUACY PROGRAM DISCIPLIPE-SPECIFIC RESULTS REPORT FORMAT CPRT (Continued) 4.3.3 Other Deviations A discussion of other deviations should be provided that describes the nature of those deviations and the bases for concluding that they were not deficiencies and that no trend exists. Any corrective action taken as o result of resolving the deviation should be discussed.

4.3.4 Observations with Adverse Trends Similar to 4.3.2 4.4 Conclusions Draw conclusions on the odequacy of the design within the subject disciplin 3.

5.0 CORRECTlVE ACTION

[

N This section discusses corrective action in a brooder context than that in Section 4. That is, this section should discuss the corrective action necessary to pres ent problems from recurring and the DAP overview of Project implemer;totion of corrective actions. This section should also describe the bases for the third party concluding that the Project has understood and w'll implement the corrective action.

l

6.0 CONCLUSION

S This section should present the conclusions drawn from the implementation of the action plan, consideration of its findings, and implementation of corrective action.
