1.0 PURPOSE This procedure provides the methods and accept / reject criteria for the performance of a documentation review for concrete placement.
r 2.0 APPLICABILITY This procedure applies to the performance of an independent review i
of the documentation for all safety-related concrete placement.
It applies to the samples selected from the population described in the Population Description for Concrete Placement.

Documentation reviews are performed and documented in accordance j
with established CPRT procedures. This procedure establishes the j
attributes and accept / reject criteria for the documentation review of Concrete Placements. Reference 3.3 addresses the method to perform and document the reviews.
4.1 Responsibilities 4.1.1 QA/QC Inspectors QA/QC Inspectors perform documentation reviews in accordance with established project procedures and instructions. Reference 3.3 addresses the method used to perform documentation reviews and to document and process the results.

5.0 INSTRUCTION ks Documentation to be reviewed is located as follows:
Inspection Reports and related laboratory test results are located in the TUGC0 Vault.
Personnel certification records are located in the TUCCO Vault /P.P.R.V.
Using the information below, perform the dccumentation reviews on the sample items listed for this population and document the results on the applicable checklist (Attachment 6.1).
Attachments 6.2 through 6.9 are examples of TUGC0 Inspection Reports, which include attributes applicable to the documentation review. Using these documents as a guide and any other available information, the inspector shall review the Inspection Reports of the sample placement to determine which attributes are applicable.
Where no inspection verification exists for applicable attributes, DR's will be initiated. Where the documentation for a given attribute is missing, lump all sub-attributes into a single Deviation Report.
1.
Design Mix, concrete batching and conveying.
Review the Concrete Production Inspection Report (see O.2 for sample) and the Concrete Placement Summary (see Attachment 6.9 for sample) and the Pour Card (see.5 for sample) or similar documents to verify notation that:
A.
Pour Card was approved and signed by the QC inspector.
NOTE: For the purpose of the checklist the " Mixed Design Cards" is the Pour Card for this attribute.
B.
Mix designated on Concrete Placement Summary corresponds to the six specified by the Field Engineer 4
on the Pour Card. NOTE: For the purpose of the l
checklist the " Mixed Design Cards" is the Concrete l
Placement Susaary for this attribute.
C.
Mix designated on pour card agrees with batch ticket.
NOTE: For the purpose of the checklist the " Mixed Design Cards-Proportions" is the six on the Pour Card for this attribute.

Batch plant scales are in calibration. This attribute i
is acceptable if it is recorded on a daily basis (day i
pour was made) prior to 7/25/77 and after 7/12/78.
Between 7/25/77 and 7/12/78 it is acceptable on a monthly basis (calendar month pour was made).

Batch ticket (see Attachment 6.3 for sample) properly filled out for receipt and add water after delivery.
Verify that when water is added to truck, data shown on.9 does not exceed that allowed on.3.
F.
Truck mixers required to complete 70 revolutions at mixing speed before start of delivery. This item is applicable to concrete batched in trucks only.
G.
Trucks are discharged before 45 minutes if ambient temperature is above 85'F, or 90 minutes if approved by the engineer.

Forms, Construction Joints Review the Concrete Preplacement Inspection Report (see.4 for sample) and the pour card (see Attachment 6.5 for sample) or similar documents, to verify that notations have been made that:
A.
Form work was installed with consideration for stability, line and grade, tightness of joints and cleanliness.

B.
Proper installation of construction joints, clean and properly wetted.
C.
Seismic air gap is in compliance with design drawings, and protected from intrusion of foreign objects.
D.
Pour card is signed off by craft and engineers, with a unique pour number.

Rebar, Cadwelds, waterstops, anchor bolts, embedded plates Review the Concrete Inspection Report (see Attachment 6.6) or similar document to verify notation that:
A.
Rebar is correct regarding size, spacing, concrete to rebar coverage, and properly installed.
B.
Cadweld/ lap splices meet design requirements.
C.
Dowels are installed with correct anchorage length.
D.
Anchor bolts are the correct type.

F.
Correct embedment has been installed.

Depositing and Consolidation Review the Concrete Inspection Report (see Attachment 6.4 for sample) or similar document, to verify notation that:
A.
Placement rate ensures proper integration with preceding lift.
B.
Placement lift does not exceed 18".
C.
Placement technique causes minimum segregation.
D.
Placement placed to proper grade.
E.
Vibrators operate at a minimum 8,000 I.P.M.
F.
Vibrators not used to convey concrete more than 3 ft.
G.
Vibrators penetrate approximately 4 in. into preceding lift.

In-Process Concrete Tests Review the Concrete Placement Summary (see Attachment 6.9 for sample) or similar document to verify notation that:

Air content is recorded and meets the acceptance criteria shown on the Brown & Root Concrete Design Mix Tabulation located in the TUGC0 vault.

Slump is recorded and meets the acceptance criteria on Brown & Root Concrete Design Mix Tabulation located in the TUGC0 vault.
C.
Concrete temperature is recorded and meets the acceptance criteria shown below:
Cold Weather Placement

NOTES: Inspector's name and required certification shall be recorded on the deviation report when a deviation l

If any initials and/or signatures are illegible, an attempt shall be made to determine the inspector's name by checking the signature cards located in the Permanent Plant Records Vault. If identification cannot be made the inspector shall reject this attribute.
