Page 3, Section 5.0, after Item 5.3 Add:

5.4 Pipe Bend Wall Thickness Verify that minimum pipe bend wall thickness meets the minimum acceptable wall thickness as indicated by the QA/QC Discipline Engineer on the " Pipe Bend Wall Thickness Record", Attachment 6.6.

The Discipline Engineer shall indicate on the " Pipe Bend Wall Thickness Record" the location (s) on the bend where Ultrasonic Testing (UT) will be performed.

NOTE: The pipe bend wall thickness-will be determined by Ultrasonic Testing (UT) performed by B&R in ;

accordance with B&R Procedure QI-QAP-10.2-4A, " Ultrasonic Digital-l Thickness Measurements".

The area of the bend where the UT will be performed shall be free of dirt, dust, debris, or paint. An CPRT QA/QC Inspector shall accompany the B&R inspector for reinspection to verify:

1, the location on the bend where thickness measurement is taken.

2. the location for UT is free of dirt, dust, debris or paint.
