Q INSTRUCTIONS FOR COMPLETING FORMS CPP-005.3A and CPP-005.3B f

" POPULATION ITEMS LIST" The Responsible QA/QC Discipline Engineer enters:

(1) Population name.

(2) Page number information.

(3) Name as preparer and date.

(4) a. Under the heading " Identification", reference the list '

which is attached by specific title and with such a description (s) that describes it in detail if it is an -

assembly of several existing lists.

b. Under the heading " Population Source", describe the population items selected, or reference an attached list, (O and identify the source of the Population List.

c. Under the heading " Basis for Accepting the List", provide the basis for accepting the list as valid, e.g., the list identifies construction completed, inspected, and QC accepted safety-related groups of similar components or activities having common attributes that are verifiable by similar reinspection and document review.

NOTE: Should the population be identified as a result of a deficiency evaluation, reference and attach the sample expansion plan. (See Reference 3.2.)

d. Under the heading " Basis for Accepting any Additional Items", identify any additional items added to the Population Items List which are not included in the above sources and provide the basis for adding the items.

The QA/QC Lead Discipline Engineer and the QA/QC Engineering Supervisor review Population Items Lists to ensure that they are complete, accura'te, and cot.sistent with the requirements of this procedure. Upon the completion of satisfactory reviews, the reviewers:
