  • CPP-020 Rev. 2 1.0 PURPOSE This procedure describes the method of reporting and processing observations by Review Team personnel which are outside the scope of the CPRT Issue-Specific Action Plans (ISAP).

2.0 APPLICABILITY

  • This procedure is applicable to hardware constructed at Comanche Peak Steam Electric Station, Unit 1, 2, and areas common to both units.

4.0 GENERAL

  • Out-of-scope observations by Review Team personnel will be reported to TUGCO. Copies of observations and any applicable responses are O retained by the Hardware Collective Evaluation Engineer.

4.1 Responsibilities 4.1.1 All QA/QC Review Team Personnel All Review Team personnel are to report out-of-scope observations by three-part mencrandums whenever they are observed.

4.1.2 Hardware Collective Evaluation Engineer The Hardware Collective Evaluation Engineer retains a copy of each out-of-scope observation and any response for information and consideration in the collective evaluation activity.

4.1.3 CPSES QC Coordinator The CPSES QC Coordinator enters out-of-scope observations in the TUGC0 NCR System and advises the QA/QC Hardware Collective Evaluation Engineer of the disposition of the NCRs.

() 4.2 Policy Review Team personnel should note apparent defects and discrepancies that are not applicable to the specific area or item being reinspected / reviewed.

4.3 Definition 4.3.1 Out-of-Scope Observation An apparent defect or discrepancy which is outside the scope of any CPRT ISAP, or relates to an action plan population but is not part of the randomly selected sample.

5.0 PROCEDURE 5.1 Initiatina Out-of-Scope observations 5.1.1 Should any Review Team personnel observe an apparent defect or discrepancy which is out-of-scope, they are '

to prepare a memorandum written in accordance with Attachment 6.1 or 6.2 as applicable. This memorandum shall be addressed to the CPSES QC Coordinator and

/ include, as applicable, the:

Date.

Condition observed.

Equipment / component identification including the mark or tag number.

Applicable drawing number including revision designation.

System description / code.

Location including elevation, reference to column lines, building, and room number.

Document package identification .nd vault location.

5.1.2 The originator enters the information required by paragraph 5.1.1, then signs and dates the memorandum.

5.1.3 After signing the memorandum, the originator forwards it to the Hardware Collective Evaluation Engineer.

Subsequently, the Hardware Collective Evaluation O Engineer assigns a serial number, forwards the original and a copy to the CPSES QC Coordinator for issuance of an NCR, and files a copy.

4 CP?-020 Rev. 2 5.1.4 CPSES OC Coordinator forwards dispositions of NCRs for out-of-scope observations to the Hardware Collective Evaluation Engineer. The Hardware Collective I Evaluation Engineer attaches copies of the dispositions to file copies of the out-of-scope observations.

5.1.5 The Hardware Collective Evaluation Engineer will forward copies of all out-of-scope observations to the QA/QC Engineering Supervisor for review.

5.2 Safety Significance Review The QA/QC Engineering Supervisor will provide population engineers with copies of out-of-scope observations related to their populations for review. Population engineers will determine which out-of-scope observations require review bi the Safety-Significance Evaluation Group (SSEG) and forward those out-of-scope observations to the SSEG Supervisor through the QA/QC Engineering Supervisor for review in accordance with reference 3.2.

5.3 Collective Evaluation ,

5.3.1 During the Collective Evaluation Process, the Hardware and QA/QC Program Collective Evaluation Engineers will j

O consider the NCR dispositions and safety significance evaluations of out-of-scope observations in identifying adverse trends not previously reported by ISAPs. After l discussing any adverse trends resulting from out-of-scope observations with cognizant Issue Coordinators, the adverse trends will be combined with those identified by ISAPs in collectively evaluating the quality of construction and adequacy of QA/QC.

5.4 Distribution and Control 5.4.1 The Hardware Collective Evaluation Engineer forwards the original and a copy of each out-of-scope observation to the CPSES QC Coordinator and a copy to the QA/QC Engineering Supervisor. He also maintains a copy of the dispositioned NCR with the file copy of the out-of-scope observation.

5.4.2 The QA/QC Engineering Supervisor retains copies of out-of-scope observations and identifies those requiring safety significance evaluations by memorandums to the SSEG Supervisor with copies to the Deputy Manager of Engineoring, Supervisor of Collective Evaluation and applicable Issue Coordinators.
