CONFIRMATION OF ACTION - DOCKET NOS. 50-413 AND 50-414 This letter updates our letter of June 27, 1986, and refers to a subsequent telephone conversation between Mr. R. D. Walker of this office and Mr. M. D. McIntosh of your staff concerning the transient which occurred to Catawba Unit 2 on June 27, 1986, during the loss of control room test.

Based upon the matters discussed, it is our understanding that you will complete the following items as described below for both units, as applicable,

1. Prior to proceeding above Mode 3:

! a. The licensee shall review and understand the root cause of the event and the sequence of events,

b. Each functional controller on each unit's main control panel (MCP),

Auxiliary Shutdown Panels (ASP), and Turbine Driven Auxiliary Feedwater Pump Control Panel (TDAFWPCP) will be reviewed with respect to valve position, nomenclature and units of measure to verify correct labeling.

Any changes required will be reflected in appropriate procedures and training.

c. The licensee shall conduct a review of all safety-related circuits where fuses of Bussman type FNA are used, and where the #ailure of the fuses would result in an undetected component malfunction such that the component's safety-related function would not be performed. The 4 licensee shall replace, modify, or perform a weekly visual surveillance on such fuses so that the safety-related function is not impaired by the deficiency currently associated with Bussman type FNA fuses. The  !

surveillance period may be modified after sufficient data is collected for justification and following consultation with NRC RII.

d. All licensed operators and Shift Technical Advisors assigned to shifts will receive training prior to standing shift in Mode 1 or 2. This training will include:

(1) A description of the June 27, 1986 event.

(2) A discussion of any associated Abnormal and Operating Procedure changes.

JUL 0 3 m Duke Power Company 2 (3) A discussion of all labeling and surface changes to the panels which resulted from the post-event analysis.

(4) Training in any other areas of the ASP operation where the post-event analysis showed an operator knowledge deficiency.

(5) Differences, identified in item Ib above, between operation of the Unit 1 and Unit 2 main control panel, the ASP and the TDAFWPCP.

e. All hardware malfunctions identified during the event and discussed at the exit of July 2 will be corrected;
f. Interim procedures for initiation of safety injection during operation from the ASP will be developed and implemented; and
g. Provide additional _ controls over start-up activities, to include additional prior review of test procedures and additional personnel training. Inform the Region of the additional controls.
2. The Loss of Control Room Test and all prerequisites will be completed prior to the Unit 2 proceeding above 30 percent power.
3. Review all Human Engineering Deficiencies identified in Duke turnover package dated June 9,1986 (NSM-CN 227) to develop a detailed schedule for accelerated implementation.
a. Within 30 days submit the accelerated detailed schedule to NRC Region II.
b. Within 90 days after entering Mode 1 complete all modifications not requiring hardware modification, control board cutting, or a system shutdown.
