As acresult of this inspection', a significant failure to comply with NRC s,

regulatory requirements was identified, and accordingly, 'NRC concerns relative

.'We have completed our review of these issues, and. conclude that certain

Y activities appeared to violate NRC requirements. The violation, references to pertinent-- requirements, and _ elements to be included-in your response are

' described in the enclosed Notice of Violation (Notice).

i The violation described in.the enclosed Notice involved an operator's failure w

to follow a procedure which in turn resulted in the diversion of 5,000 gallons of reactor coolant.to the Refueling Water Storage Tank.

,

Although this has been categorized as a Severity Level IV violation, it is

-

recognized that this event is but the-latest of occurrences which in the i

aggregate are cause for NRC concern relative to.the quality of.operaf :ons at Catawba.

These' concerns include but are"not limited to the topics of plant i

equipment configuration control, unnecessary challenges to plant safety

systems, as well as failure to follow procedures.
